Momentum Nutrition Fees & Insurance

Telehealth visits (via Zoom) can be a convenient way to gain support for your nutrition and wellness goals. It also allows us to work all over the world! In person visits also available in our Kenmore office.

• Initial Consultation (80 minutes) $295
• Follow Up Visits (45-50 minutes) $180

Some insurance companies cover the cost of nutrition care. Momentum is considered “out of network” and does not bill insurance companies directly. We provide each client with an itemized billing statement to submit for reimbursement. Payment is required at the time of service. We request you place a credit card on file with our billing processor Square, unless other payment arrangements have been made. Payment is through the Square App and paid invoices are sent monthly for your submittal to insurance.

Momentum offers a sliding fee scale for a limited number of clients in need, please just ask.
